POLICIES AND CANCELLATIONS

To cancel or put your EFT on hold we MUST have a COMPLETED cancellation or hold form in our possession by the 15TH of the PRIOR MONTH that you would like to cancel or go on hold. You also MUST be on EFT automatic transfer for a minimum of TWO month BEFORE CANCELING
some may have an early cancellation adjustment fee.

A joining fee  will be collected for new students and students who haven't been to Jazzercise for
6 months or longer

EFT MEDICAL HOLD POLICY: MEDICAL HOLDS may be requested and placed on your membership for up to 3 months (12 weeks) for health related reasons (surgeries, maternity leave).
Holds cannot be placed in weekly increments. Holds are NOT valid for vacation or travel.

We will be unable to refund any monies if the EFT HOLD or CANCELLATION was not received by the 15th of the month via EFT cancellation form. Please refer to your EFT agreement for more information.

FOR EXAMPLE: IF YOU WOULD LIKE TO CANCEL OR GO ON HOLD (MEDICAL REASONS ONLY) FOR FEBRUARY 1 ST WE MUST HAVE A COMPLETED FORM IN OUR POSSESSION BY
JANUARY 15TH (NO LATER)
